How Hera helps families

• Personalized referrals to vetted partners & the care coordination to ensure nothing falls through the cracks
• Medication management to ensure medications are organized, correct, and delivered
• Expert insurance navigation to understand benefits and resolve billing issues
• Medicaid planning and getting key health documents in place.
• Advocacy to organizations such as MLTC for additional approved hours for care

We meet our families through referral partners: geriatricians and social workers from health systems like Weill Cornell and NYU Langone to independent PCPs.
